Rising sixth-grade student-athletes seeking a sports physical will need to attend during the date and time assigned to the representing school they plan to attend in the coming school year.

In order to receive their physicals, participants must complete and bring all required forms with them. All forms must be signed by each students’ parents. No physicals will be performed without the necessary signed paperwork.

Appointments are not necessary. Students just need to show up during their school’s designated day and time to receive their exams. All physicals will be performed on a first-come, first-served basis.

For more information, students may contact their school’s front office for registration paperwork and any related questions.

Any student-athlete that does not participate in this free event will have to consult their primary care physician or pediatrician to obtain their sports physical evaluation.
